Panic Attacks and Agoraphobia

Presenting Issue: Recurrent panic attacks, fear of leaving the house.
The client experienced sudden panic attacks in public places (shopping centres, public transport), leading to intense fear of having another attack. This resulted in avoidance behavior—he began staying home and avoiding most social situations. He feared “losing control” or embarrassing himself.
Used Exposure-Based CBT for Panic Disorder, beginning with psychoeducation about the panic cycle. Therapy included interoceptive exposure (e.g., recreating sensations like increased heart rate), and gradual real-world exposure to feared locations. Breathing techniques and grounding tools were taught early on to build confidence.
After 12 sessions, the client began traveling short distances alone and re-engaging socially. Panic attacks significantly decreased in frequency and intensity. He reported: “I still get anxious, but I don’t let it stop me anymore.”
